I, Jagtar Singh, son of Darshan Singh, holder of Indian Passport No. F8560735, issued at Delhi, on July1/2006, permanent resident of B-9/13, Gokal Pur, Delhi 110094, and presently residing at 12 Tessler Crescent, Brampton, ON, L6X4P8, Canada, do hereby change my name from JAGTAR SINGH  to JAGTAR SINGH MALHI, for all future purposes.
