I, Harpreet singh, son of Mahal Singh, holder of Indian Passport No. H7730162 issued at Toronto, Ontario, on 10 august 2009, permanent resident of 6 B Sant Nagar Karnal, Haryana 132001, and presently residing at 38 Eiffel Blvd, Brampton L6P1W3, do hereby change my name from Harpreet Singh to Harpreet Singh Hanjra with immediate effect.
